In the qualification protocol, I prepared 2 types of attachments as below
Attachment 1) Participant registration form
Attachment 2) Proof of qualification sheet
Before starting the process, I registered the participant’s name and have them sign the 1st attachment.
And I noted the date in the protocol main page.
In this case, can I print the participant’s name and the year on the 2nd attachment before starting qualification?
or must I handwrite their names and year one by one during the qualification?
Following my company’s regulation, I must sign every page including the pictures of proof.
