Some of the worst executions I’ve seen as far as mistakes and non-compliant documentation were made by a group of people hastily put together from different departments, without training, to execute a protocol.
The people were not from the users department, nor did they belong to the validation department. They had little understanding of the needs of validation, and as their jobs did not depend on the results or compliance of their testing efforts, they JUST DID NOT CARE.
